Upon request, the AFMRD provides family medicine residency programs with Resident Award for Scholarship certificates (PDF format) suitable for printing and framing.

Award Criteria

• The award will be presented one time per year and to one resident. No resident may win the award more than once.
• Two or more residents may be eligible for a joint award for jointly performed advocacy activity. In this instance only, multiple certificates may be requested.

We will email your certificate to you within two weeks of your order.

Please enter the recipient's name exactly as it should appear on the award certificate. If a team of residents worked together on a joint project, you may request a certificate for each participating resident.
