As we prepare for our AFMRD Annual Meeting tomorrow—yes, a shameless plug—I want to thank you for the honor and privilege of allowing me to serve as your president this year. Last night several of you asked me if I am happy my term is over. In fact, I enjoyed this year. I enjoyed meeting many of you and discussing family medicine and graduate education in depth. I enjoyed helping set the strategic plan for our organization. I enjoyed speaking passionately on a national stage about our specialty, healthcare policy, and how the success of each is dependent on the other. There are many things in my life of which I am proud--family, charity, success of my graduates--and I count this year among them.

I leave AFMRD in the capable hands of Dr. Karen Mitchell. I will remain dedicated to our cause and will assist her as I am called and able. I will never stop speaking up or out as I feel appropriate. I hope you can agree that AFMRD has been and continues to be the best organization for family medicine residency directors. Please remain engaged, remain enthusiastic in the face of adversity, and continue to do the good works you do every day.

These changes include many of those supported by the AFMRD, and other family medicine organizations, through testimony and feedback.

The AFMRD’s Board of Directors has started a discussion with the other members of the Council of Academic Family Medicine (CAFM) about the possibility of creating a Standardized Letter of Evaluation, or SLOE, to be used by medical students showing interest in applying to family medicine residency programs.

For those of you not familiar with the SLOE, a form of it is currently being utilized by emergency medicine residency programs. All applicants are required to have at least one SLOE completed by a faculty member at an EM residency program. The purpose of this requirement was multi-factorial. First the SLOE is standardized so that it contained specific information deemed important and sought by program directors and the format was easy to use even by those not gifted in the art of letter of recommendation writing. Second it was hoped it would better delineate a student’s actual interest in their specialty as they needed to seek out a particular emergency medicine faculty member who knew them well enough to complete the SLOE. For more details on the EM SLOE visit the website: www.cordem.org. We understand that general surgery is also developing a similar process and other specialties may be considering this as well.

Clearly there are many questions about such a process or requirement for family medicine residency programs. What information would the form be seeking? Who could complete it? Would it be required? Is it legal? What is the airspeed velocity of an unladen swallow? Among others.

The Board would like to gauge interest from our membership before working on such a task with our colleagues in CAFM. Please take a moment to complete a one-question poll. We can discuss this further at our Annual Meeting on March 25, if there is sufficient interest.

In response to the concerns raised by our membership, the AFMRD and the ADFM sent a joint letter to President Trump asking for a re-evaluation of his executive order regarding immigration.

We have already heard how this order has affected medical students and resident physicians and recognize this will harm their well-being and the well-being of the patients and communities they serve. We hope the current stay gives time for either reversal or major revision of this order.

I just spent three very productive and positive days at the AFMRD January Board meeting. The January meeting is one of three face-to-face meetings the Board holds each year. We connect via hour-long conference calls each of the other nine months.

As an AFMRD Board member and now president, I have had the opportunity to meet many of you in person as well as had substantive conversations through the community page and email with many more. Being a member of the Board has been truly rewarding work. Board members work on issues important to program directors, advocate for graduate medical education and our specialty, and collaborate with other organizations for the betterment of health care. I am proud that I have been a part of such an organization’s leadership.

Today, we opened the nomination period to recruit new Board members.

I would encourage you to seriously consider standing for the AFMRD Board of Directors. This is a unique opportunity to make a difference on a national stage, work with others who are passionate about education and policy that affects our profession, and help to preserve the foundations of family medicine training. In addition, Board members are integral to developing tools directors can use to improve their programs. Plus it is a lot of fun.

The deadline to apply is Monday, February 20, 2017.

In the midst of the holiday season I want to wish you all a joyous and happy new year. As I look out my window at the picturesque snow covered pines and cedars highlighted by a bright blue sky, listening to U2, awaiting my first patient of the day I reflect on how lucky we are and how far we have come.

As a resident physician I would hear my faculty discuss battles they had in board rooms and hospital hallways about privileges. No, not whether they could perform ultrasound guided biopsies, but rather could they perform an admission history and physical or read an ECG or have access to an x-ray of one of their patients.

Patient-centered care then meant the patient sat surrounded by a team of specialists arguing whose mentors went to a more prestigious university. Today, we speak about what additional services we can provide to care for our patients as we put them front and center.

We speak of evidence and choosing wisely, not tenure and wizened appearances. We prevent diseases our grandparents fought at best to mitigate. Better still we measure the lives of patients with conditions like AIDS and cardiomyopathy in decades not days. We train an energetic and talented group of young physicians who see technology as a partner and not an adversary and whose sense of service reminds me of our forefathers’.

The road we travel now has lights that use less energy but burn brighter than ever. But it is still a rough road in spots and our destination is still ill-defined and may yet need to be built. But build it we will. Together. With our patients, our families in mind. We have no other course to take but the one that leads us forward. Where we find road blocks, we will build bypasses. Where we find division, we will build bridges. Where we find no road at all, we will blaze new trails. And where we find a traveler who is less fortunate, abused or misguided, we will give our charity. Just as we have always done.

Thank you for all the good you do in your communities every day. To you and your families I wish you peace and warmth and health. We all deserve at least that to keep on our journey.

As the holiday is upon us, I wanted to wish each of you and your families a Happy Thanksgiving. I also want to ask you to find the things in your life for which you are thankful. For me that is family, friends, success, health, and life. Lately it seems harder to appreciate such things openly. We are leaving a contentious and often mean spirited election cycle (now well over what used to be an election year) and face the challenges of an uncertain future in a new administration. Recruitment season is long and increasingly more complex. Resident physicians can seem more entitled (didn’t our program directors say that about us?). Daylight hours are less and paperwork in a paperless society is more.

But in spite of all of that, we persevere. We bring hope to the hopeless, health to the infirmed, healing to the sick, and comfort to those who would otherwise be lost. In the recent, vibrant discussion of the number of applications submitted by students, forgotten was the remaining data published by the AAMC. Currently there are 103,000 family physicians providing direct patient care in the United States, more than every other specialty including internal medicine and dwarfing most other specialties. A full 14% of America’s physician workforce was trained in our programs. Most who receive care do so from our graduates. So over the next few days reflect on what you have done. Remember the all babies you helped bring into the world and all the individuals you calmed as they left it. Remember the troubled resident physician who now serves the underserved. Remember the great things and good works you do every day. Remember what is important and be thankful.

On behalf of the Board of your AFMRD I want to take this opportunity to thank all program directors and to bid farewell to those of you who will be leaving our collective group while welcoming those who join us. As I said in my president’s address, we do remarkable work. Through the care of family physicians who have trained under our leadership, our body has the potential to positively impact over 300 million people. Those are real lives, not just statistics on spreadsheet. Our graduates are there when they are born, praise them at their first words, splint them when they fall, coach them as they start a family, pray with them as they morn a loved one, comfort them as they struggle with illness, laugh with them for no reason at all, and hold their hand as leave this world for the next. No other specialty—and perhaps no other profession—has such an impact.

Thank you all for everything that you do every day and for everyone for whom you care, directly or indirectly. What greater good can there be? For those of you who are leaving us, we wish you well in wherever your life’s journey leads you, hoping you find time to let us know how you and your families are doing. Thank you for the positive impact you have made on so many lives.

For the new members of our ranks, we want to tell you what a great job this is. While its rewards are often not readily apparent, you need only to look at your community to know that great things are being done. And when times are hard, and they will be, this organization of your peers is here to support you, comfort you, build you a splint, or simply hold your hand when needed. And at all other times we are here to laugh with you for no reason at all. Welcome.

In May, a Prez Release discussed the alternative payment model known as CPC+ or Comprehensive Primary Care Plus. In that release we informed our membership that this pilot program would involve 20 geographic populations.  The Centers for Medicare and Medicaid announced the names of 14 regions:  Arkansas, Colorado, Hawaii, Kansas City Metro (KS/MO), Michigan, Montana, New Jersey, New York (Hudson Valley), Ohio (includes Northern Kentucky counties), Oklahoma, Oregon, Philadelphia Metro, Rhode Island and Tennessee.  We still believe that the rules allow residency programs in these eligible regions to apply as part of this initiative.  Interested program should first visit the CMS FAQ on CPC+ to determine if their payer mix is part of the program.  For more information on CPC+ see these resources: http://www.aafp.org/practice-management/transformation/cpci/cpc-plus.html and https://innovation.cms.gov/Files/x/cpcplus-faqs.pdf.

Important update -- just in time for interview season!

In July, your AFMRD Board of Directors wrote a letter to the National Residency Matching Program (NRMP), expressing concern about recent changes to The Match rules, in particular the following:

For all Matches opening after June 30, 2016, programs are prohibited from requesting applicants to reveal the names, specialties, geographic locations, or other identifying information of programs to which they have or may apply.

We recently received a positive response to our letter. We appreciate that the NRMP took the time to consider AFMRD members’ concerns and to offer some clarification and reassurance to program directors.

To ensure your interviews are as productive as possible, please take time to review the NRMP letter prior to scheduling and meeting with prospective residents.

Thank you to all members who responded to the survey on pain management and opioid prescribing. As you may recall, this survey was developed by AFMRD at the request of AAFP President Dr. Wanda Filer, who will cite the survey results when discussing the opioid epidemic with legislators and others. She has shared the data with to the U.S. Surgeon General. You will find the survey results in the AFMRD PD Toolbox. The AFMRD Board of Directors hopes to talk more about this important issue at the 2017 PDW and RPS Residency Education Symposium.

At this year’s Program Directors Workshop, Dr Peter Carek announced that the ACGME would be evaluating citations given to programs prior to the Next (New) Accreditation System and that they would be removing citations from programs that had been in substantial compliance over the past two years. This month you may be receiving notifications of these removals if your program has received no new citations or extended citations due to the past two annual reviews.

Program Directors had asked for a review of ‘old’ citations as it was deemed to be of little worth to continue to respond to them annually if the deficiency had already been corrected. We thank the ACGME and our family medicine review committee for this positive change.

The AFMRD,  in collaboration with the ACOFP, developed the Accreditation Navigation course.  This course is an intensive two-session program modeled after AFMRD's renowned National Institute for Program Director Development (NIPDD). The Accreditation Navigation Course, taught by experienced faculty, covers all aspects of the ACGME accreditation process.

Registration for the 2016-2017 Accreditation Navigation course is now open. The deadline to register is August 1. If you are interested, don’t delay. Seats are limited. There’s also a free webinar open to anyone. See details.

During this year’s AFMRD Annual Meeting members were informed that our organization completed the strategic objectives outlined in our strategic plan and will begin developing a new strategic plan in the fall.

In order for the staff and board to properly take on this endeavor we need input from our members. Last week you should have received an email asking you to complete a short survey for our members’ needs assessment. I ask each of you to take a few minutes to thoughtfully complete this survey before June 22nd.

It was through your input three years ago that we built the foundation for current programs including the Residency Curriculum Resource, the Residency Program Index, the Graduate Survey and the Accreditation Navigation Course. It also helped influence the board as to how best to utilize the resources of our association to serve our members well.

Thank you in advance for completing the survey and for all you do in the name of family medicine education. Please help us make the next three years as productive as the last.

You may be hearing about CPC+ or Comprehensive Primary Care Plus. CPC+ is a Centers for Medicare and Medicaid Services initiative for a payment structure that rewards value and quality and supports comprehensive primary care. It follows and expands upon the initial CPC initiative which ends this December. Currently CMS is accepting applications from payer sources to determine 20 geographic regions across the country where primary care practices will be eligible to participate. Once these regions are determined practices can apply to be a part of one of two tracks for payment reform. The anticipated application period will start on July 15, 2016. Currently there is no language that would prohibit a primary care residency practice from applying.

CMS hopes that CPC+ will help to meet the goal that at least 50% of fee-for-service primary care will be shifted to an alternative payment model by 2018. This is just one of those payment models. It’s that time of the year when we start spring cleaning, preparing for the arrival of our new resident physicians, and, for many of us, renewing our RCR subscriptions. The Residency Curriculum Resource is a joint venture between AFMRD and the Society of Teachers of Family Medicine. It provides a free online overview of the core curricula in family medicine including a peer-reviewed reading list, and a subscription based repository for peer-reviewed case based presentations. These presentations include quizzes and facilitator guides for ease of use within your program. There are currently more than 100 presentations in a variety of topics. The RCR is also a great way for your faculty and resident physicians to meet their scholarly activity requirements while showing off their expertise on a national stage. Currently there is a call for authors on a wide group of topics. At the AFMRD annual meeting I encouraged each of us to enlist a faculty member to contribute to this great body of work. If each program only took on one subject, we would have a national database of over 450 up-to-date presentations. Chances are your program already has a fantastic presentation ready to go. Single Form for Verification of Graduate Medical Education Training

Recently the American Hospital Association announced the creation of a single form for use by all agencies needing verification of an individual’s graduate medical education training.  This was a joint effort of the AHA, ACGME, National Association of Medical Staff Services and the Organization of Program Director Associations (OPDA).  AFMRD had input through our liaison to OPDA, Dr. Deb Clements.   Discussions have been held with the Federation of State Medical Boards, with the hope they will incorporate this into their verification system as well.

Many of us have been frustrated by having to fill out several verification forms for a single individual often unknown to the certifying program director.  This form will allow for completion at graduation from training and then to be re-used as requested.  For individuals who left training prior to this year, completion could be done at the next request and then re-used thereafter.

While the form does not include all the information often requested by medical staff offices, such as procedure lists, it does serve its intended purpose, making at least one aspect of credentialing easier.  AFMRD applauds this work and hopes to continue to support efforts to simplify processes such as this.  A copy of AHA’s announcement and the verification form can be found here on our website.  As more information becomes available we will continue to share it with our members.
